Job Summary:

The Case Manager II will work directly with assigned clients to develop and implement individualized service plans, ensuring they receive the necessary support and resources to achieve their goals.

Key Responsibilities:
  • Conduct comprehensive assessments to identify client needs and develop tailored service plans.
  • Collaborate with community partners and resources to provide clients with access to essential services and benefits.
  • Maintain accurate and up-to-date case documentation, adhering to Catholic Charities' standards and regulatory requirements.
  • Provide education and advocacy to clients, empowering them to make informed decisions about their lives.
  • Transport clients as needed, ensuring safe and reliable transportation.
  • Consult with supervisors and team members to ensure seamless client care and coordination.
Requirements:
  • Bachelor's degree in human services or a related field, or equivalent experience.
  • Valid driver's license and proof of insurance for transporting clients.
  • Ability to work with diverse populations, including those experiencing homelessness, unstable housing, and mental illness.
  • Additional training or education may be required based on program needs.
Physical Requirements:
  • CPR certification.
  • Ability to lift up to 25 pounds without assistance.
  • Ability to lift up to 50 pounds without assistance.
  • Ability to physically restrain clients, if necessary.
