Job Summary:The Case Manager III will work directly with assigned clients to develop and implement personalized service plans, ensuring their needs are met and they receive the necessary support to thrive.
Key Responsibilities:- Conduct initial and annual assessments to identify client needs and develop tailored service plans.
- Provide education and case management services to clients, including in-home visits.
- Support clients in accessing community resources and benefits, and assist with required paperwork.
- Collaborate with community systems to facilitate linkage, referral, crisis management, and follow-up.
- Maintain accurate and complete case documentation and records.
- Coordinate with supervisors and team members to ensure seamless client care.
- Utilize trauma-informed, person-centered, and solution-focused techniques to promote personal growth and minimize crisis.
- Mentor other Case Managers within the program.
- Perform other duties as assigned.
- Bachelor's or master's degree in social work, psychology, sociology, human services, or a related field.
- At least one year of experience as a social worker/case manager/care coordinator in a public or private social services agency.
- Pass DHS background check and meet their requirements for the provision of case management.
- Ability to work with diverse and/or low-income populations, including those experiencing homelessness, unstable housing, and/or mental illness.
- Valid driver's license and proof of current insurance coverage if transporting clients.
