Job Summary

Pinnacle Services is seeking a skilled Case Manager to join our Hennepin/Chisago County team. As a Case Manager, you will work with individuals with developmental disabilities, serious mental illness, or chemical dependency issues, providing personalized support and connecting them with community resources.

Key Responsibilities
  • Assess individual needs and develop personalized service plans
  • Coordinate and provide resources for individuals, including in-home care, assisted living, and case management
  • Monitor service delivery and work with providers to ensure individual needs are met
  • Attend regular meetings with individuals and their interdisciplinary teams
  • Document client-related work and create billable units
Requirements
  • Graduate degree in social work, psychology, sociology, or a related field
  • At least one year of experience working with individuals with developmental disabilities
  • Valid driver's license and acceptable driving record
  • Successful completion of local and/or state background checks
