We are seeking a highly skilled and compassionate professional to join our team as an Intensive Case Manager - Housing Support Specialist. This is a unique opportunity to make a meaningful difference in the lives of chronically homeless individuals.
About UsAffordable Living for the Aging (ALA) is a nonprofit organization dedicated to eradicating senior homelessness and early institutionalization. Founded in 1978, we provide affordable housing options and ancillary services to low-income and homeless seniors throughout Los Angeles.
Job SummaryThis role will deliver intensive case management services to assist older adults in achieving independence, mental and physical health, and emotional well-being. The successful candidate will work closely with clients to develop personalized care plans, ensuring seamless linkage to healthcare, mental health, and substance use disorder services.
Key Responsibilities- Evaluate client mental health status, history of substance abuse, medical, psychosocial, environmental, legal, financial, education, strengths, and needs
- Develop and update care plans every three months, incorporating new goals, progress, and changes
- Link clients to accessing health, mental health, and substance use disorder services as needed
- Assist clients with maintaining medication and treatment regimens, including accompanying them to appointments
- Help clients obtain income, establish benefits, and locate employment or educational opportunities
- Support clients in maintaining daily independent living, social/leisure, and personal hygiene skills
- Assist clients with budgeting, money management, credit restoration, and eviction prevention
- Provide eviction prevention counseling, advocacy, and intervention strategies
- Bachelor's or Master's degree in Social Work or Mental Health related field preferred
- At least one year of experience working with the homeless population or two years providing direct mental health or intensive case management services
- Experience working with clients with mental illness, chronic health issues, and substance use disorders
- Expertise in outreach and engagement strategies, intensive case management services, crisis intervention, and affordable housing
- CHAMP experience preferred
- Competitive salary range: $60,000 - $80,000 per annum
- Reliable transportation and successful completion of background screening required
- Covid-19 vaccination or valid exemption, CA Driver's License, and proof of insurance coverage necessary
- Proof of degree certificates and/or license certification, and experience verification required
