About the Role

We are seeking a highly motivated and compassionate Mental Health Case Manager to join our team at SCHARP California. As a key member of our staff, you will provide intensive case management services to adults with severe and persistent mental illness who are homeless or at risk of hospitalization.

Job Responsibilities

  • Link clients to community resources such as housing, medical, educational, and vocational services.
  • Provide rehabilitation services that focus on improving daily functioning through an integrated recovery approach.
  • Assist clients in completing necessary housing applications and working collaboratively with housing specialists.
  • Work as a collaborative team member with inter and intra-agency staff to assist in stabilizing clients.
  • Provide intensive crisis intervention services 24/7 to clients at risk of hospitalization or incarceration.
  • Maintain regular face-to-face contact with assigned clients and ensure they are involved in establishing goals.
  • Complete periodic outcome measures with clients.

Requirements

  • A Bachelor's Degree in Social Work, Psychology, or a related field (preferred).
  • At least two years of experience in mental health/social services (with a BA degree), four years with an AA degree, or six years with an HS Diploma.
  • A valid California Driver's license and reliable transportation with adequate insurance.
