Job Title: Registered Nurse

We are seeking an experienced Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will be responsible for collaborating with healthcare professionals to provide high-quality patient care and ensuring a seamless transition of care across the continuum.

Key Responsibilities:
  • Collaborate with healthcare professionals to develop and implement patient care plans
  • Provide education and support to patients and their families
  • Coordinate care transitions to ensure continuity of care
  • Monitor quality metrics and ensure accurate documentation in patient records
Requirements:
  • Graduation from an accredited school of nursing and current licensure in the State of New York
  • Minimum of 8 years' experience in adult health condition management
  • Baccalaureate Degree in the Science of Nursing (BSN) degree from an accredited school of nursing preferred
