Job Summary:

The Patient Care Coordinator will oversee the complex process of patient bed assignments to meet care needs and maximize bed resources. This includes patients from external sources, emergency departments, clinics, post-operative care, and elective procedures. The coordinator will be responsible for patient transfers within both hospitals, ensuring seamless care transitions. This role will unify bed management to optimize capacity, reduce emergency department diversion, and minimize boarding hours.

Key Responsibilities:

Develop and implement bed management strategies to meet patient care needs

Collaborate with multidisciplinary teams to ensure efficient patient transfers

Analyze data to optimize bed resource utilization

Requirements:

NYS Licensed/Registered RN (or eligible) required. Bachelors degree in Nursing and three years post-licensure acute care RN experience (preferably in Emergency Services or Critical Care) required. Excellent communication, organizational, and critical thinking skills required. Must be able to work collaboratively with a multi-disciplinary team. CPR-BLS certification required.

Preferred Qualifications:

Acute care experience in emergency services or critical care is preferred. Leadership experience preferred.

Work Schedule:

Variable shifts, including nights, weekends, and holidays
