We are seeking a skilled and compassionate Registered Nurse to join our team as a Weekend Home Health Nurse. As a Weekend Home Health Nurse, you will provide high-quality patient care and support to patients in their homes, working closely with our interdisciplinary team to ensure seamless care coordination.
Key Responsibilities- Provide skilled nursing visits to patients in their homes, assessing their needs and developing individualized care plans to promote optimal health and well-being.
- Conduct thorough assessments, develop and implement care plans, and provide ongoing care and support to patients and their families.
- Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care coordination and patient outcomes.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions and patient satisfaction.
- Document patient care and progress accurately and thoroughly, maintaining up-to-date records and reports.
- Diploma, Associate, or Bachelor's degree in Nursing.
- Minimum of one year of experience in skilled Medicare home visits.
- OASIS and HCHB experience required.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
