Job Summary:

We're seeking a Registered Nurse (RN) to join our team and deliver high-quality, patient-centered care in the comfort of patients' homes. As a Home Health Registered Nurse, you'll be responsible for:

Key Responsibilities:

  • Providing admission, case management, and follow-up skilled nursing visits for home health patients.
  • Administering ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
  • Confer with physicians in developing the initial plan of treatment based on physician's orders and initial patient assessment.
  • Implementing patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems.
  • Coordinating and overseeing the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers.
  • Monitoring the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
  • Discharging patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.

Requirements:

  • Diploma, Associate, or Bachelor Degree in Nursing.
  • Minimum of one year nursing experience in Skilled Home Visits required.
  • Home Care Home Base Experience a Plus.
  • OASIS proficiency required.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.

CenterWell Home Health offers a competitive compensation package, including a base pay range of $69,800 - $96,200 per year, paid time off, 401(k) retirement savings plan, employee assistance program, business travel, and accident benefits.
