Position: Case Manager

Reporting To: CUA Case Management Supervisor

Location: CUA Cares Center

General Purpose:

The Case Manager plays a crucial role in delivering ongoing support and services to designated children and families. This position is responsible for the formulation and execution of comprehensive service plans based on thorough assessments of client needs. The Case Manager will also guide families in developing effective parenting strategies that mitigate safety risks while fostering a nurturing environment for children.

Key Responsibilities:

  • Conduct intake assessments to identify the needs of families.
  • Perform home, school, and community visits to evaluate needs and assist families in accessing necessary resources in accordance with established policies and regulations.
  • Develop a detailed service plan in alignment with organizational policies.
  • Facilitate access to additional resources as required.
  • Collaborate with other service components within youth and family services.
  • Maintain accurate client records and ensure timely completion of all required documentation.
  • Provide on-call services as needed.
  • Prepare written and/or oral reports for relevant authorities as necessary.
  • Engage in mandatory training sessions.
  • Participate in supervision and professional development activities.
  • Perform other related duties as assigned.

Minimum Qualifications:

  • Bachelor's degree in Social Work or a related field such as sociology, psychology, counseling, criminal justice, education, divinity, or public health administration.
  • Two years of experience in case management, particularly with youth.
  • Valid driver's license with a clean driving record.

ADA Considerations:

  • This position requires regular hearing, speaking, walking, and sitting; occasional standing, driving, and lifting up to 25 pounds. Regular travel is also required.
