Position Overview:
The Case Manager plays a vital role in delivering essential in-home support to vulnerable families and individuals identified through referrals. This position requires a commitment to confidentiality and the ability to provide assistance beyond standard working hours.

Case Manager Details:
  • Schedule: 8:00 AM - 5:00 PM, with hybrid options post-training. Monday to Friday, plus one Saturday each month.
  • Compensation: $41,000 - $54,000 annually, commensurate with experience.
  • Employment Type: Direct Hire Role
  • Benefits: Generous PTO, tuition reimbursement, signing bonuses, and retirement matching programs.
Case Manager Responsibilities:
  • Deliver in-home services to at-risk families or individuals as determined by referrals, managing a diverse caseload that includes both low and high-risk cases.
  • Conduct case management activities, including home visits, family assessments, service plan development, crisis intervention, advocacy, resource connection, and maintaining case files in accordance with program standards.
  • Collaborate with educational staff, social workers, and service providers to monitor progress and effectively link families to necessary resources.
  • Offer services and support outside of regular hours to meet the needs of families, including being available for on-call emergencies as needed.
  • Participate actively in staff meetings, supervision, training sessions, and case reviews.

Case Manager Qualifications:
  • Minimum of 2 years of experience in case management or a related field.
  • Experience in navigating public benefits for clients.
  • Familiarity with collaborating with educational personnel.
  • Bachelor's degree is mandatory.
  • Mandated Reporter Certificate (obtained within the last 5 years).
