Position Overview:
The Case Manager plays a vital role in delivering essential in-home support services to vulnerable families and individuals identified through referrals. This position requires a commitment to confidentiality and the ability to provide assistance beyond standard working hours.

Case Manager Details:
  • Schedule: 8:00 AM - 5:00 PM, with hybrid options available post-training. Monday to Friday, plus one Saturday each month.
  • Compensation: $41,000 - $54,000 annually, commensurate with experience.
  • Employment Type: Direct Hire Position
  • Benefits: Generous Paid Time Off, Tuition Reimbursement, Signing Bonuses, and Retirement Matching Programs.
Key Responsibilities:
  • Deliver in-home services to families or individuals at risk, based on referrals, managing a diverse caseload that includes both low and high-risk cases.
  • Execute case management tasks, including conducting home visits, performing family assessments, developing service plans, managing crisis situations, advocating for clients, connecting them to necessary resources, and maintaining case files in accordance with program standards.
  • Collaborate with educational staff, social workers, and service providers to communicate progress and effectively link families to available resources.
  • Offer support and services outside of regular working hours to meet the needs of families, including being available for on-call emergencies as needed.
  • Participate actively in staff meetings, training sessions, supervision, and case reviews.
Qualifications:
  • A minimum of two years of experience in case management or a related field.
  • Experience in navigating public benefits for clients.
  • Familiarity with working alongside educational personnel.
  • A Bachelor's degree is mandatory.
  • Must possess a Mandated Reporter Certificate (obtained within the last five years).
