Job Overview

Caring People Alliance is seeking a dedicated Case Manager to join our team at the CUA Cares Center. This full-time position is essential in providing ongoing support and services to children and families, ensuring that comprehensive delivery plans are developed and implemented based on thorough assessments of client needs.

Key Responsibilities:

  • Conduct initial intakes and assessments to identify family needs.
  • Perform home, school, and community visits to evaluate needs and assist families in accessing necessary resources in accordance with established policies and regulations.
  • Develop and implement a detailed service plan as per organizational guidelines.
  • Facilitate access to additional resources when required.
  • Collaborate with other components of youth and family services to ensure a holistic approach.
  • Maintain accurate client files and complete all required documentation promptly.
  • Provide on-call services as needed.
  • Prepare written and/or oral reports for relevant authorities as necessary.
  • Engage in mandatory training sessions and supervision meetings.
  • Perform other related duties as assigned.

Minimum Qualifications:

  • Bachelor's degree in Social Work or a related field such as sociology, psychology, counseling, or public health administration.
  • At least two years of experience in case management with youth.
  • Valid driver's license with a clean driving record.

Physical Requirements:

  • This position requires regular hearing, speaking, walking, and sitting; occasional standing, driving, and lifting up to 25 pounds. Regular travel is necessary.
