Hospice Nurse Case Manager

Insight Global is seeking a skilled Hospice Registered Nurse Case Manager to join our team. As a key member of our healthcare staff, you will be responsible for delivering high-quality patient care and ensuring the effective execution of care plans.
Key Responsibilities:
- Provide comprehensive nursing care to hospice patients, including assessing their needs and developing individualized care plans.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Utilize your clinical expertise to identify opportunities for improving patient outcomes and quality of care.
- Travel to patient homes within a 50-mile radius of the designated area, utilizing your personal vehicle for transportation.
Requirements:
- 2+ years of clinical RN experience, with a strong background in hospice care.
- Active RN license in Louisiana.
- Dependable vehicle, driver's license, and insurance.
Preferred Qualifications:
- Hospice experience, with a proven track record of delivering exceptional patient care.
Compensation:
The targeted salary for this role is $71-$75,000 per year, plus a $2,500 sign-on bonus.
