Work Schedule: Monday to Friday, 8 AM to 5 PM, with two holidays on call each year.

Key Responsibilities:
  • Manage a caseload of patients and families, overseeing assessment, eligibility determination, admission processing, care planning, implementation, and evaluation.
  • Gather and assess critical information regarding the patient's physical, psychological, social, and spiritual needs, initiating interventions as necessary.
  • Facilitate communication with attending physicians, interdisciplinary teams, and external agencies to ensure coordinated care.
  • Conduct regular visits to reassess patient needs and update care plans accordingly.
  • Perform clinical pronouncements of death and provide compassionate support to families.
  • Maintain accurate patient records, including charts, progress notes, and timely care plans.
  • Supervise and document the delivery of care by Home Health Aides in compliance with federal, state, and company regulations.
Qualifications:
  • Graduate from an accredited nursing program.
  • Current registered nurse license in the state of practice.
  • Certification in CPR and First Aid is required.
  • Knowledge of quality assessment and assurance processes is essential.
  • A valid driver's license and proof of insurance are mandatory.
