Job Summary

The RN Case Manager Lead will supervise a team of RN Case Managers in the inpatient and emergency departments at all NCH campuses. The successful candidate will have strong leadership and team-building skills, possess excellent communication skills and problem-solving abilities.

Essential Duties and Responsibilities
  • Support the Director of Case Management and Social Work Manager in implementing and enforcing NCH and departmental policies and procedures.
  • Supervise, orient, and mentor RN Case Managers.
  • Reinforce compliance with department documentation standards of initial comprehensive assessments, readmission assessments, progress, and discharge notes.
  • Ensure comprehensive individualized discharge planning that meets the needs of patients and their families.
  • Identify gaps in services and develop strategies to improve patient care and resource utilization.
  • Participate and promote quality of care initiatives, length of stay, readmissions, and avoidable days.
  • Provide patients/families with resources to meet their medically related social and emotional needs.
  • Ensure safe transition of our patients from one care environment to another.
  • Promote patient/family involvement and ensure choice in discharge planning.
  • Collaborate with multi-disciplinary teams, healthcare providers, and community partners to ensure comprehensive patient care and care transition.
Requirements
  • Minimum of Associate Degree in Nursing required; BSN preferred.
  • A minimum of 2 years of supervisory/lead/manager level of experience in hospital Case Management is required.
  • Licensed as a Registered Nurse in the state of Florida.
  • Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
  • Case Management Certification Preferred.
  • Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
