About the Role

Gentiva Hospice is seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN. As a Hospice RN, you will provide high-quality patient care and support to individuals and their families during a challenging time.

Key Responsibilities
  • Provide direct patient care services to hospice patients and supportive care to the patient's family
  • Act as primary liaison between physician, patient/family and hospice team
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
  • Maintain all assigned patient clinical records
  • Participate in company quality assessment and performance improvement programs
Requirements
  • RN license in the state of requested employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
  • Current driver's license and automobile insurance
  • Current CPR certification
