Job Summary

The successful candidate will be responsible for coordinating patient care plans, managing length of stay, ensuring resource utilization, and developing safe discharge plans in collaboration with the multidisciplinary team. This role requires strong clinical skills, excellent communication, and a patient-centered approach.

Responsibilities:

  • ">
  • Coordinate activities to promote quality outcomes and patient throughput while supporting optimal care and resource utilization">
  • Provide case management services for both inpatient and observation patients as assigned">
  • Identify patients at risk for adverse outcomes during transitions from one level of care/setting to another">
  • Perform comprehensive assessments of psychosocial, medical, and discharge needs of patients/families along with resources available">
  • Reassess patients' clinical conditions as indicated, considering readmission status or risk and developing strategies to mitigate">

Requirements

  • ">
  • 2+ years' experience in case management or 3+ years' experience in clinical nursing">
  • InterQual experience preferred">
  • Associate Degree in Nursing or Nursing Diploma required; Bachelor's Degree in Nursing preferred">
  • Certification in Case Management preferred">
  • Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation">
