Healthcare Coordinator for Adults with Developmental Disabilities

Job Summary
We are seeking a skilled RN Health Care Coordinator to provide support services to individuals residing in our community group homes. As a key member of our team, you will be responsible for the oversight of the overall healthcare and well-being of our residents.
Responsibilities
- Provide consultation regarding the medical needs of our residents
- Train staff on specific treatment protocols
- Ensure effective communication and collaboration with our team
Requirements
- Current Massachusetts license as a Registered Nurse (RN)
- At least 3 to 5 years' experience in a community health setting
- Valid driver's license and reliable transportation
