Job Summary

Stormont Vail Health is seeking a skilled Licensed Practical Nurse to join our team. As a Clinical Care Specialist, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare organization.

Key Responsibilities:

  • Assist with patient check-in, examinations, and procedures.
  • Administer prescribed medications and treatments, clarifying medication orders and refills as directed by providers.
  • Triage patient telephone calls, evaluating the physical and psychosocial health status of patients.
  • Coordinate patient services, including lab tests, point-of-care testing, x-rays, diagnostic procedures, consults, home care, and acute care admissions.
  • Provide patient education on medications, lab tests, x-rays, and procedures, notifying patients of test results and provider disposition.
  • Document medical information using electronic applications and/or forms.
  • Assess patient rooms and surrounding areas for safety issues, removing hazards and making adjustments as needed.
  • Communicate patient safety and patient care issues to appropriate parties.
  • Maintain cleanliness of equipment, examination, and treatment rooms.
  • Maintain exam rooms with necessary supplies and materials.
  • Participate in Patient Safety Rounds.
  • Adhere to infection control, emergency, safety, fire, and disaster plans and policies, completing required initial training and updates per hospital policy.
