Job Title: Outreach Care Specialist

We are seeking a highly motivated and compassionate Outreach Care Specialist to join our team at Elevance Health. As a key member of our care coordination team, you will play a vital role in ensuring that our members receive the highest quality care and support.

Job Summary:

The Outreach Care Specialist is responsible for coordinating follow-up care plans for members, assessing their compliance with medical treatment plans, and identifying opportunities to improve their health and meet quality goals. You will work closely with our members, healthcare providers, and community resources to ensure that our members receive the care and support they need.

Key Responsibilities:
  • Coordinate follow-up care plans for members, including scheduling appointments and enrolling them in programs.
  • Assess member compliance with medical treatment plans and identify opportunities to improve their health.
  • Work with healthcare providers and community resources to ensure that our members receive the care and support they need.
  • Identify and address barriers to care, including social determinants of health.
  • Develop and implement care plans that meet the unique needs of each member.
  • Collaborate with our care coordination team to ensure that our members receive comprehensive care and support.
Requirements:
  • High school diploma or equivalent required; 1 year of related experience preferred.
  • Behavioral health experience strongly preferred.
  • Familiarity with community resources and social determinants of health preferred.
  • Community health worker, certified nurse assistant, or certified medical assistant experience preferred.
  • Bilingual candidates preferred.
