About Our Program - Homeless Family Transition Specialist

Eckerd Connects' Family Support Services is a leading organization that provides vital assistance to children and families struggling with homelessness and unemployment. In partnership with the Brevard County School District and local agencies, we offer case management, counseling, and parent training services to help families access community resources, secure housing, and achieve stability.

Job Summary - Homeless Family Transition Specialist

We are seeking a highly skilled and compassionate Homeless Family Transition Specialist to join our team. As a Family Coach, you will work closely with families to identify their unique needs and create personalized plans to address homelessness and transition to stability. Your expertise will be invaluable in connecting families with essential resources, providing career coaching, and promoting sustainability in their new homes.

Key Responsibilities - Homeless Family Transition Specialist
  • Conduct thorough needs assessments to develop tailored Service/Performance plans and progress reports for each family.
  • Engage families in goal-oriented skills training to reduce harm to the family unit and mitigate risk, while referring them to resources that support their finite goals.
  • Establish and maintain ongoing contact with families, conducting home visits to measure and promote sustainability once they have acquired housing.
  • Keep up-to-date client files and case notes on all contact and activities related to cases, completing required tracking and follow-up data collection.
Requirements - Homeless Family Transition Specialist
  • Master's Degree in a related human service field or equivalent experience is required. A minimum of two years' experience in a related field and/or training, or an equivalent combination of education and experience, is also necessary.
  • A valid state driver's license and ability to meet requirements for Eckerd's Auto Insurance are essential. You must also be able to drive for business purposes.
  • A background screening process, including state, local, and funder criminal background checks, and a drug screen in compliance with our Substance Abuse and Drug-Free Workplace policy, are required.
