Role Overview

As a Nurse Practitioner, you will collaborate with an interdisciplinary team to help patients and their families make decisions that honor their deepest values. By understanding their beliefs, we customize our care to enhance their quality of life, fostering peace and fulfillment.

Responsibilities
  • Collaborate with an interdisciplinary team to provide comprehensive care to patients and their families
  • Assess patients' needs and develop personalized care plans
  • Provide education and support to patients and their families
  • Participate in ongoing education and training to stay up-to-date on best practices
  • Work collaboratively with other healthcare professionals to ensure seamless care
Requirements
  • 2 years of Nurse Practitioner experience in an acute care environment
  • Masters of Science in Nursing - Family Nurse Practitioner
  • Active unencumbered state NP license
  • Current driver's license and auto liability coverage
  • Drug Enforcement Agency (DEA) number Required
  • National Provider Identifier (NPI) Required
