Hospice Care Specialist

**Job Summary:**
We are looking for a skilled and empathetic individual to join our team as a Hospice Aide. As a Hospice Aide, you will provide direct patient care to patients under the direction of the RN to ensure optimum care.
**Responsibilities:**
* Provides personal care for each patient in accordance with the Plan of Care
* Contributes to developing the Hospice Plan of Care by reporting observations to the RN Case Manager
* Observes, informs, and documents changes in the patient's physical and emotional condition to the Interdisciplinary Team
* Respects professional boundaries while providing care in facilities and patient homes
* Demonstrates safe techniques in all transfers and ambulation
* Provides emotional support to patients and caregivers
**Requirements:**
* Must have reliable transportation, a valid driver's license, and willingness to travel within the office's service area
* Current CNA (Certified Nursing Assistant) and/or HHA (Home Health Aide) Indiana State certification
* Willingness to serve on the on-call rotation schedule
* The ability to perform the physical duties of this position
* Ability to manage your schedule effectively and efficiently

Guardian Angel Hospice is a locally owned and operated hospice care organization serving 30 counties with offices in Kokomo, Carmel, Lafayette, and Logansport. We offer nursing, aide, social, chaplain, bereavement, and volunteer services along with medical equipment, supplies, and medications (related to the diagnosis) for people in the last phase of an incurable disease so that our patients live as fully and comfortably as possible.
With appropriate hospice care and a community sensitive to their needs, patients and their families are free to attain a degree of mental and spiritual preparation for death, making its coming more comfortable for them.
