Physiocare Hospice Registered Nurse

We are seeking a skilled Hospice Registered Nurse to join our team at Fortis Health. As a key member of our care coordination team, you will be responsible for planning, coordinating, and providing high-quality patient care to our patients and their families.
Key Responsibilities- Conduct thorough assessments of patients' needs and develop individualized care plans to ensure optimal outcomes.
- Collaborate with interdisciplinary teams to coordinate patient care and services.
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and regulatory requirements.
- Current state licensure as a Registered Nurse.
- One year of experience in home health or hospice care, or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
