The Home Health and Hospice Intake Coordinator will be responsible for managing and facilitating the transition of patients, maintaining timely and accurate records, and reporting to the Director of Intake and Service Development. The ideal candidate will have at least 1 year of recent acute experience in a hospital setting, with a strong preference for those with home health or hospice experience.
