Job Title: Home Care Intake Coordinator RN

VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Care Intake Coordinator. This role is responsible for assessing patients' home health care needs, coordinating referrals to home care and hospice, and providing education and strategies for patients and families.

Responsibilities:
  • Participate in a two-week clinical orientation to learn key procedures, the EMR system, compassionate care, and safety.
  • Assess and evaluate the needs of patients with multiple disease processes and comorbidities, and develop an appropriate plan of care.
  • Provide education and strategies for patients and families.
  • Coordinate referrals to home care to promote continuity of care.
  • Partner with other VNA clinicians and external providers to execute best patient outcomes.
  • Document all changes in patient's condition and update care plans and outcomes.
Qualifications:
  • RN; Bachelor's degree in Nursing preferred.
  • LPN; Graduate of an NLN accredited LPN program with current NJ LPN licensure.
  • Valid NJ nursing license.
  • 2+ years of community health nursing experience.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal and organizational skills.
  • Strong client focus, assertive, decision-making skills, articulate, and creative in problem-solving.
  • Strong decision-making skills.
Working Conditions/Physical Demand:

This role is based in a fast-paced healthcare environment with phone, scanner, email, fine motor hand movement, and computer use. The ideal candidate will have a valid driver's license and insurance to use their personal automobile for work-related purposes.
