About the Role

Parker Jewish Institute for Health Care and Rehabilitation is seeking a skilled RN-Home Care Admissions Nurse to join our team.

This key clinical role involves ensuring that Parker's programs and services are effectively represented to patients, families, and others who have an interest in home care through pre-assessments, information sharing, and admissions in a patient's home.

The Admissions RN works closely with the intake team, referring physician, and other referral sources to promote the CHHA philosophy of care. Assessments may take place in private homes, hospitals, or nursing facilities.

Responsibilities
  • Ensure effective representation of Parker's programs and services to patients, families, and others
  • Work closely with the intake team, referring physician, and other referral sources to promote the CHHA philosophy of care
  • Conduct pre-assessments, information sharing, and admissions in a patient's home
  • Assessments may take place in private homes, hospitals, or nursing facilities
Requirements
  • Current New York State RN license
  • BSN a plus
  • 1-2 years of experience as an RN
  • Experience in a Home Care, Hospice, or Community Health environment strongly preferred; experience as an intake RN a plus
  • Proficient in EMR software; familiarity with Allscripts a plus
  • Access to reliable transportation; ability to conduct home visits as needed
  • Vaccinated against COVID-19
