As a key member of our clinical team, you will be responsible for overseeing the day-to-day operations of our onsite and remote intake operations, ensuring effective triaging of new referrals and admissions, and reinforcing the implementation of new initiatives.
Key Responsibilities:- Supervise and coordinate the work of the Hospice Intake team, ensuring that all aspects of the Hospice Liaison Intake process are met.
- Establish and maintain relationships with customers and referral sources, ensuring effective communication and collaboration.
- Ensure that the care directed to acute symptom management is provided within the standards of practice for all clinical disciplines.
- Collaborate with Quality Improvement/Education to develop and implement quality improvement and educational activities to ensure professional practice standards are consistently met.
- Communicate effectively with VNS Health Care management and hospice agencies under contractual relationship with VNSNY Hospice Care.
- Perform all duties inherent in a supervisory role, including staff training, interviewing candidates for employment, evaluating staff performance, and recommending hiring, promotions, and terminations.
- License and current registration to practice as a Registered Professional Nurse in the state of New York.
- Valid driver's license or NYS Non-Driver photo ID card, may be required as determined by operational/regional needs.
- Bachelor's Degree in Nursing.
- Minimum of three years of progressive clinical experience in home care, Hospice or related field.
- Demonstrated knowledge of the regulatory and reimbursement requirements governing Hospice.
