Care Coordinator RN Specialist

CommonSpirit Health is seeking a highly skilled Care Coordinator RN to join our team. As a key member of our care coordination team, you will play a critical role in overseeing the progression of care and discharge planning for patients requiring these services.
The ideal candidate will have a minimum of two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing. A BSN or related healthcare field is preferred, as well as at least five years of nursing experience.
We offer a competitive salary range of $83,000 - $110,000 per year, depending on experience. In addition to your annual salary, you may be eligible for bonuses and other benefits.
As a Care Coordinator RN at CommonSpirit Health, you will have the opportunity to work with a multidisciplinary team to ensure seamless transitions of care and optimal outcomes for our patients.
