Job Summary

We are seeking a skilled Registered Nurse Case Manager to join our team at CenterWell Home Health. As a key member of our home health team, you will provide high-quality patient care and case management services to patients in their homes.

Responsibilities
  • Provide skilled nursing visits for home health patients, including admission, case management, and follow-up.
  • Administer ongoing care, case management, and evaluation of care plan for each patient.
  • Coordinate appropriate care with various healthcare personnel and report patient care/condition/progress to the physician and Clinical Manager.
  • Supervise Home Health Aides, Certified Home Health Aides, and Personal Care Workers.
  • Discharge and prepare appropriate medical documentation for patients.
Requirements
  • Diploma, Associate, or Bachelor Degree in Nursing
  • At least one year of nursing experience preferred
  • Strong med surg, ICU, ER, or acute experience
  • Home Health experience is a plus
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
  • A valid driver's license, auto insurance, and reliable transportation are required.
