Position Overview

The Community Support Specialist plays a vital role in facilitating access to and the upkeep of high-quality rehabilitative support services for individuals. This position involves providing direct assistance and fostering consumer autonomy through the creation of Personalized Recovery Plans, setting objectives, and tracking advancements, while ensuring seamless care continuity.

KEY RESPONSIBILITIES

  • Adhere to regulatory standards and organizational policies.
  • Encourage wellness and recovery initiatives by applying evidence-based practices.
  • Oversee a designated caseload and evaluate individual needs.
  • Record observations and relay pertinent information effectively.
  • Offer guidance to consumers and support daily living tasks.
  • Promote engagement with family and community support systems.
  • Supervise medication administration and maintain accurate tracking.
  • Engage in group activities and community outreach.

Administrative Duties

  • Prepare necessary documentation for consumer records.
  • Accurately complete billing documentation in a timely manner.
  • Report any facility-related issues and manage consumer funds as directed.
  • Participate in required training sessions and special initiatives.

SKILLS AND QUALIFICATIONS

  • Exhibit sound judgment and discretion in various situations.
  • Possess strong communication and analytical thinking abilities.
  • Proficient in reading, writing, and speaking English; bilingual skills are advantageous.
  • Effectively manage multiple tasks while demonstrating cultural awareness.

EDUCATIONAL REQUIREMENTS

  • Bachelor's degree in mental health, human services, or equivalent experience.
  • Previous hands-on experience in the field is preferred.
  • Familiarity with Microsoft Office and technology usage.
  • Certification in First Aid and CPR is required.
  • A valid driver's license and adherence to driving record policies are necessary.

PHYSICAL DEMANDS

  • Utilization of standard office equipment and household items.
  • Physical effort may be required for assisting consumers and lifting tasks.
