Community Bridges, Inc. is a leading provider of integrated behavioral healthcare services in Arizona. We are seeking a skilled Housing Liaison to support individuals in need of eviction prevention services.

Job Summary

The Housing Liaison will work closely with internal and external agencies to establish relationships and facilitate referrals for individuals eligible for eviction prevention programs. This role requires strong case management skills, excellent communication, and a passion for supporting vulnerable populations.

Key Responsibilities
  • Establish and maintain relationships with internal and external agencies to facilitate referrals and support services
  • Conduct thorough assessments to identify individuals eligible for eviction prevention programs
  • Develop and implement individualized plans to support housing stability and prevent evictions
  • Collaborate with finance department to process and deliver eviction prevention funds
  • Maintain accurate records and reports to track program effectiveness
Requirements
  • High school diploma or equivalent required; associate's degree or higher in a field related to behavioral health preferred
  • Minimum 6 months experience in behavioral health and/or substance abuse field required
  • Lived experience in recovery from mental health and/or substance use disorders, homelessness, family members, and veterans preferred
  • Current AZ driver's license in good standing
  • Clear 39-month motor vehicle record
  • Behavioral health technician (BHT) certification in accordance with A.A.C. R and CBI clinical policies and procedures
  • Certified peer support specialist (CPSS) preferred
