Job Summary

The PRN Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who provides high-quality, patient-centered care in a fast-paced ambulatory setting. This role requires strong clinical expertise, excellent communication skills, and the ability to work effectively in a team environment.

Key Responsibilities
  • Manage patient flow, triage, and direct patient care to ensure efficient and effective delivery of services.
  • Serve as a liaison between front desk staff, nursing staff, physicians, and clinic manager to facilitate communication and coordination of care.
  • Develop and implement patient care plans that address the physical, psychosocial, financial, and environmental health care needs of patients.
  • Provide education and support to patients and families to promote self-management of disease and improve health outcomes.
  • Coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
  • Manage high-risk patient care, including patients with multiple co-morbidities or high risk for readmission to a hospital setting.
  • Develop relationships with patients and families to identify and address their health care needs.
  • Utilize electronic tools, such as EMR, to efficiently document assessments, clinical findings, plans of treatment, and progress toward goals.
Requirements
  • High School Graduate
  • Bachelor Degree Nursing (BSN) required within 5 years for existing employees in role
  • 2 or more years RN experience
Preferred Qualifications
  • Previous medical office experience
License and Certification
  • Basic Life Support Heartsaver (BLS HS) - American Heart Association (AHA) or obtained within 14 days
  • Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing Licensed in the state in which they will be working and proof of compact licensure submitted/applied for in the state in which the employee lives
