As a RN Case Manager, you will play a vital role in ensuring timely and beneficial care to our customer-owners while adhering to established case management practice standards and Advance Access principles.

**Key Responsibilities:**
* Deliver high-quality patient care through effective assessment, planning, implementation, and evaluation of individualized care plans.
* Collaborate with multidisciplinary teams to ensure seamless coordination of care and promote patient-centered outcomes.
* Utilize evidence-based practices and clinical expertise to make informed decisions that improve patient health and well-being.

**Requirements:**
* Graduate of an accredited nursing program with a valid Registered Nurse license in the State of Alaska.
* Basic Life Support (BLS) certification required; ACLS, NRP, ATLS, ALSO, and PALS certifications required for specialty.
* One year of nursing experience or successful completion of the RN Case Manager Training Program at SCF.
