Position Overview

La Frontera Center Inc. is seeking a compassionate professional to deliver vital support to children and families experiencing challenges. In this role, you will be instrumental in crafting, assessing, and managing treatment strategies and recovery initiatives.

Main Responsibilities:
  • Build respectful and goal-oriented relationships with clients receiving assistance
  • Create thorough clinical evaluations and service plans with contributions from the child, family, or recovery team
  • Oversee and execute service plans in accordance with State regulations
  • Keep precise clinical records and document progress accurately
  • Deliver services through various interactions and support systems
  • Evaluate clients' responses to services and collaborate on necessary adjustments
  • Lead meetings to review progress and identify needs
  • Coordinate with external organizations for additional resources
  • Ensure transportation arrangements to essential services
  • Record all interactions and services rendered
Requirements:
  • Bachelor's or master's degree in a relevant field, or equivalent professional experience
  • Understanding of Managed Care systems and provider networks
  • Experience in case management and behavioral health initiatives
  • Valid driver's license and dependable transportation
  • Bilingual (Spanish/English) abilities preferred
