About the Role

We are seeking a highly skilled and compassionate Intake Assessor to join our team at Foundations for Living. As a key member of our admissions process, you will play a critical role in ensuring that our patients receive the highest level of care and support.

Responsibilities
  • Conduct thorough risk assessments to identify the most appropriate level of care for potential patients
  • Coordinate the referral and intake process, facilitating communication with referral sources, families, patients, and unit staff
  • Work closely with the Business Office to verify patient financial data and prior authorization of insurance benefits
  • Collaborate with the UR Department to ensure accurate certification of patients admitted
Qualifications
  • Master's Degree in a human relations or other related field
  • At least two years of experience in intake assessment activities within an acute psychiatric setting
  • Licensed to practice in the State of Texas as a mental health professional (RN, LMSW, LCSW, LPC or LPCI)
