Position Overview:
The Acute Care Case Manager (CM I) is essential in enhancing the synergy between clinical practices and financial frameworks to foster positive health outcomes.

Key Responsibilities:
Accountable for evaluating, integrating, and facilitating patient discharge strategies, with a focus on coordinating care for medium to high-risk patients throughout the healthcare spectrum.

The primary objective is to ensure that discharged patients have a "reasonably safe discharge plan," engaging available support systems when permitted by the patient.

This role guarantees the provision of cost-effective, outcome-driven, and high-quality care within an anticipated length of stay. It involves managing patient care to avert fragmentation and duplication of services.

Collaborative Practice:
Works in conjunction with physicians and other healthcare professionals to address patient-specific and age-related needs, linking cost and quality to the care provided.

This position ensures that care is delivered at the appropriate level and setting.

Qualifications:

  • Preferred qualifications include an RN or Social Work degree (MSW or BSW).
  • A degree in human services with a minimum of 2 years of direct care experience is acceptable.
  • Experience in home care, hospital, or nursing home settings is strongly preferred.

Licensure and Certifications:

  • Must maintain current licensure and/or certification with the relevant professional body.

Compensation:
The expected base pay for this position is between $32.00 and $58.74 per hour, determined on a case-by-case basis, taking into account experience, education, and relevant certifications.
