The Psychiatric Case Manager plays a pivotal role in the development and execution of a patient's treatment and discharge strategy. Operating under the guidance of the Nurse Manager within the Inpatient Behavioral Health Unit, this professional collaborates with a multidisciplinary team, including the patient, psychiatrist, nursing staff, and program coordinators, to create a thorough and effective treatment and discharge plan.
Key Responsibilities
- Conduct comprehensive assessments of the patient's care plan immediately upon admission, with regular evaluations at least weekly or as necessary.
- Take the lead in patient discharge planning, identifying potential barriers promptly to prevent delays.
- Ensure all discharge documentation is completed accurately and within set timeframes.
- Compile a detailed social history for each patient and maintain appropriate records.
- Actively participate in daily multidisciplinary treatment team meetings.
- Serve as a liaison for the patient between the unit and external agencies.
- Facilitate family meetings to evaluate and determine discharge requirements.
- Ensure all legal documentation regarding the patient's mental health status is accurately completed.
- Coordinate with external organizations to ensure timely and suitable post-discharge plans.
Qualifications
- A Bachelor's Degree is required; a Master's Degree is preferred.
- A minimum of two years of experience in acute care case management is preferred.
- Experience in psychiatric case management and inpatient psychiatric settings is highly desirable.
Licenses and Certifications
- Must maintain current licensure and/or certification relevant to the professional field.
