Key Responsibilities
  • Coordinate medical care and match patient needs with healthcare resources
  • Work collaboratively with physicians and other healthcare professionals to deliver cost-effective, outcome-oriented care
  • Ensure the delivery of quality care within an appropriate length of stay
  • Manage patient care to prevent fragmentation and duplication of services
Requirements
  • 3-5 years of experience in care management or a related field
  • NYS PRI Certification strongly preferred
  • Experience working with adult and geriatric populations
