Job Summary

We are seeking a highly skilled and experienced Home Care Manager, RN to join our team at Hudson Headwaters Health Network. As a key member of our interdisciplinary team, you will play a vital role in developing and implementing care plans for our frail elderly population.

Key Responsibilities
  • Assess and Develop Care Plans: Conduct thorough assessments of participants' needs and develop individualized care plans in collaboration with contracted home care providers.
  • Coordinate Care Services: Provide direct oversight and management of participants' home care needs, ensuring timely and accurate documentation of services provided.
  • Lead Interdisciplinary Team Meetings: Facilitate IDT meetings and provide leadership for effective care planning and problem-solving for quality participant services.
  • Audit and Coordinate Home Care Agencies: Ensure all regulatory trainings, certifications, and health assessments are completed and up-to-date for contracted staff.
  • Review Medical Records: Ensure timely and accurate documentation is readily available for participants' medical records.
  • Collaborate with Participants and Families: Work with participants and families to ensure an appropriate plan of care is developed to meet all home care needs.
  • Coordinate Appointments and Services: Work with contracted providers, participants, and families to coordinate appointments and services.
  • Assist Clinical Leader: Assist the Clinical Leader in ensuring the provider plan of care is executed and documented, and that all necessary diagnostics are completed in a timely manner.
Requirements
  • Education and Experience: Associates degree in nursing required, Bachelor's RN degree preferred. 2+ years of geriatric nursing expertise and 4+ years of supervisory healthcare experience.
  • Licensure and Certifications: Current New York State RN License, BLS certified, and certified to complete PRI and UAS-NY assessments.
  • Skills and Abilities: Excellent interpersonal, verbal, and written communication skills, ability to work within a clinical team, and experience working with a diverse population of colleagues and participants.
Physical Requirements
  • Ability to Safely Perform Job Functions: Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA, and other federal, state, and local standards.
  • Physical Demands: Requires the exertion of 50 pounds of force occasionally, up to 20 pounds of force frequently, and up to 10 pounds of force constantly to move objects. May also need to move heavy equipment, transfer/lift patients, and assist patients with walking and turning.
