Brio Living Services is seeking a highly motivated and compassionate Service Coordinator to join our team.
The ideal candidate will have a strong background in social work and experience working with older adults and/or non-elderly disabled individuals.
As a Service Coordinator, you will play a critical role in identifying, assisting, and linking residents to the supportive services needed to continue living safely in an independent setting.
Key Responsibilities- Coordinate supportive services for residents, including general case management and referral services.
- Evaluate resident needs using assessment tools and interact with residents, families, and others to provide direction and resources.
- Liaise with service agencies and volunteers to find individualized, flexible, and creative services for residents.
- Educate residents on service availability, application procedures, and their rights.
- Develop, organize, and maintain supportive services programs within retirement communities.
- Manage department budget and document monthly case notes and annual submission of standard of process report.
- Bachelor of Social Work or equivalent work experience required.
- 2 years of experience in social service delivery with older adults and/or non-elderly disabled required.
- Ability to use computers, including Microsoft Suite, required.
- Make a meaningful impact in the lives of older adults and ensure a gracious, worry-free retirement is available for everyone.
