Job Summary

Key Responsibilities
  • Assign responsibility for completing sections of the Minimum Data Set (MDS) to staff who have clinical knowledge about the patient.
  • Assure that non-licensed staff who provide direct care to the patients are allowed to contribute information for the assessment process.
  • Conduct, complete, or coordinate other disciplines in the completion of the RAI and certify the completion of the Assessment.
  • Sign and certify the Care Area Assessment (CAA) Summary Form.
  • Attend care planning and enter the care plan decision information.
  • Assist with month-end duties for closeout.
  • Perform other assignments as assigned.
Requirements
  • Graduate from an approved school of nursing, college, or university.
  • Current, unencumbered license to practice nursing as a Registered Nurse/Licensed Practical Nurse in Florida.
  • Minimum of two years of RAI and Care Plan experience, preferably in a Skilled Nursing Facility (SNF).
  • Knowledge of MDS 3.0, including Assessments, CAA's, and Care Plans.
  • Ability to organize work procedures, assume responsibility, and tactfully interact with other disciplines.
  • Excellent communication and interpersonal skills.
Working Conditions
  • Must be able to sit, stand, bend, twist, lift, and move intermittently throughout the day.
  • Must be able to lift, push, pull, and otherwise move a minimum of 50 pounds.
  • Able to see and hear adequately in order to respond and interact with other staff and families.
  • Able to speak in a clear, concise voice in order to communicate with patients, families, and other staff and disciplines.
