About This Hospice Response Team LPN Position

HopeHealth is seeking a Hospice Field LPN to join its affiliate HopeHealth Hospice & Palliative Care. The Response Team LPN will respond to urgent patient needs, requiring excellent clinical assessment skills and the ability to quickly identify the best course of action to treat patients.

This is a full-time 40 hour night position that includes a weekend and holiday rotation of every-other weekend, (1) major and (1) minor holiday. The hours for this position are 11:00am-7:30pm and are eligible for benefits. This position is eligible for a sign on bonus up to $2,500.

Key Responsibilities:

  • Implement the individualized plan of care and recommend revisions to the plan as necessary and plan for discharge/transfer as appropriate.
  • Consult with and educate the patient/family/caregiver regarding the disease process and teach the patient/family/caregiver self-care techniques, as appropriate. Provide medication, diet and other instructions as ordered by the physician.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Coordinate all patient/family services and prioritize needs with the members of the interdisciplinary team to develop and revise patient's individualized plan of care.
  • Inform the physician and other personnel of changes in the patient's needs and outcomes of intervention.
  • Determine scope and frequency of services needed based on acuity and patient/family needs.
  • Coordinate care with other interdisciplinary team members in cooperation with the family/caregiver to meet identified needs of the patient/family/caregiver; including psychosocial, spiritual, and emotional needs and refer to appropriate disciplines/services.

Requirements:

  • Minimum of 1-3 years of clinical experience.
  • Excellent assessment, clinical and communication skills.
  • Bilingual Preferred but not required.
