Job Title: Rapid Response Licensed Practical Nurse

About the Role:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Hospice Response Team. As a member of our team, you will respond to urgent patient needs, providing exceptional clinical care and support to patients and their families.

Key Responsibilities:

  • Implement individualized plans of care, recommending revisions as necessary, and planning for discharge or transfer as appropriate.
  • Consult with and educate patients, families, and caregivers regarding disease processes, teaching self-care techniques, and providing medication, diet, and other instructions as ordered by physicians.
  • Initiate preventive and rehabilitative nursing procedures, coordinating patient and family services, and prioritizing needs with the interdisciplinary team to develop and revise patient plans of care.
  • Inform physicians and other personnel of changes in patient needs and outcomes of interventions, determining scope and frequency of services needed based on acuity and patient needs.
  • Coordinate care with other team members, in cooperation with families and caregivers, to meet identified needs of patients and families, including psychosocial, spiritual, and emotional needs, and refer to appropriate disciplines and services.

Requirements:

  • Strong desire to join an organization committed to providing exceptional patient-centered care.
  • Licensed as a LPN in Rhode Island and Massachusetts.
  • Minimum of 1-3 years of clinical experience.
  • Excellent assessment, clinical, and communication skills.
  • Bilingual preferred but not required.
