The Social Worker provides social work services to primary care patients in participating Vermont Blueprint for Health Program and participates as an active member of the Community Health Team.
Key Responsibilities:
• Two years of social work experience in a community, home health, or outpatient practice setting are required.
• Knowledge of various local, state, and federal resources is necessary.
• Expertise in advanced care planning is strongly preferred.
• Experience with interventions for adults, families, and children is strongly preferred.
Requirements:
• Valid license in social work, vocational rehabilitation, substance abuse counseling, or a related field is required.
• A valid driver's license and auto insurance are also required.

Work Schedule: 8:00 AM to 5:00 PM;
