Job Summary: BAYADA Home Health Care is seeking a skilled Clinical Care Specialist for a nursing job in Vermont. The ideal candidate will have a current license as a Registered Nurse in Vermont, a minimum of one year of recent, verifiable clinical experience, and strong interpersonal skills.
Responsibilities:
- Develop and implement designated care plans in accordance with patient needs
- Visit clients in designated geographic territories to provide medical care and support
- Administer medication, wound care, treatments, and procedures as assigned
- Monitor client conditions and report changes to Clinical or Client Services Manager
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided

Requirements:
