Social Work Specialist

The Social Worker provides critical social work services to patients in participating Vermont Blueprint for Health Program, working collaboratively as an integral member of the Community Health Team.
Key Responsibilities- Deliver high-quality social work services to patients in a community, home health, or outpatient practice setting.
- Utilize knowledge of local, state, and federal resources to support patients.
- Develop and implement advanced care plans, leveraging expertise in this area.
- Engage in interventions with adults, families, and children, applying strong interpersonal skills.
- Maintain valid licensure in social work, vocational rehabilitation, substance abuse counseling, or a related field.
- Hold a valid Driver's License and Auto Insurance.
Requirements:
- Two (2) years of social work experience in a community, home health, or outpatient practice setting.
- Strong knowledge of local, state, and federal resources.
- Expertise in advanced care planning highly preferred.
- Experience with interventions with adults, families, and children highly preferred.
