Job Overview

Sea Mar Community Health Centers, a Federally Qualified Health Center (FQHC), is a community-based organization dedicated to providing quality healthcare services to diverse communities.

We are recruiting for the position of Referral Tracking Specialist, responsible for tracking provider referrals to specialist and imaging facilities, ensuring timely communication with patients and specialists, and maintaining accurate documentation in the electronic health record.

Responsibilities
  • Monitor and track referrals processed by Medical Assistants to ensure timely follow-up and completion
  • Close loops on referrals with patients, specialists, and electronic health record
  • Maintain accurate and up-to-date records of referrals and patient interactions
Requirements
  • Bilingual proficiency in English and Spanish required
  • High school diploma or equivalent required
  • Experience in medical office setting preferred
  • Proficiency in business machines, including fax, phone system, EPIC, and Microsoft Office
