Key Responsibilities- Provide professional nursing care to patients in their homes, focusing on comfort, dignity, and well-being.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Conduct thorough assessments, identify patient needs, and coordinate care with other healthcare professionals.
- Administer medications, treatments, and therapies as prescribed by physicians.
- Monitor patient progress, report changes, and adjust care plans as needed.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and coordination.
- Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
- Document patient information, progress, and outcomes in accordance with regulatory requirements and organizational standards.
- Diploma in Nursing, Associate's Degree in Nursing, or Bachelor's Degree in Nursing.
- Current licensure as a registered nurse with the North Carolina Board of Nursing.
- Compact license may apply, but licensee should confirm with NCBON.
- BCLS certification.
- Valid Driver's License.
- National certification in area of specialty (preferred).
- One year of acute care experience.
